Whatever form you fill out or document you attach your signature on is a document that speaks of who you are. The name, age, and other demographic information you provide is a mirror of you. And the person reading your forms sees you on what you put on the forms.This is actually one of the basic aspect of forms on personal level but it has an even heavier weight that most of us sometimes neglect or take for granted, its legal bearing.
I'm not a lawyer but I'm sure that we all know the saying that "Honesty is the best policy." And in providing your information, the same is expected. Otherwise, you will be guilty of falsification of public documents.
